1. Setup > Company > General Preferences > DEFAULT CUSTOMER TYPE = Individual
2. Type field is not exposed on the form selected during the CSV Import (In Step 2 : Import Options > Advanced Options > CUSTOM FORM).
To resolve the problem:1. Navigate to Customization > Forms > Entry Forms.
2. Edit the custom form selected on Import Advanced Options.
3. Click Fields tab > Main > enable the Show check box for Type field.
4. Click Save.
